Lazy Dog Guest Survey Sweepstakes — Official Rules

NO PURCHASE NECESSARY. A purchase or payment will not increase your chances of winning.

1. Eligibility. The Lazy Dog Guest Survey Sweepstakes (the “Sweepstakes”) is open only to legal residents of the fifty (50) United States and the District of Columbia who are at least eighteen (18) years old at the time of entry. Employees of Lazy Dog Restaurants, LLC (“Sponsor”) and its parents, affiliates, subsidiaries, advertising and promotion agencies, and the immediate family members and household members of each such employee are not eligible. Void where prohibited. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations.

2. Sponsor. Lazy Dog Restaurants, LLC, 101 Shipyard Way Suite B, Newport Beach, CA 92663.

3. Promotion Period. The Sweepstakes begins at 12:00:01 a.m. Pacific Time (“PT”) on September 15, 2026 and ends at 11:59:59 p.m. PT on September 30, 2026 (the “Promotion Period”). Entries must be received during the Promotion Period to be eligible. Sponsor’s computer is the official time-keeping device for the Sweepstakes.

4. How to Enter. There are two ways to enter. Regardless of entry method, limit one (1) entry per person for the entire Promotion Period. Entries in excess of this limit are void. All entries must be received by the end of the Promotion Period to be eligible for the drawing.

4(a). Survey entry. During the Promotion Period, an entrant may enter by scanning the promotional QR code displayed on the checkout screen at a participating Lazy Dog Restaurants & Bar location and then completing and submitting the guest survey in full, including all information requested on the entry form. A completed survey and entry form is required for a survey entry to be eligible. Entries may be tracked using the applicable QR code, UTM parameters, or other campaign parameters. Internet access is required to enter by survey; standard data rates may apply.

4(b). Mail-in entry. To enter without completing the survey, handprint in English in black ink on a 3” x 5” piece of paper the entrant’s complete name, mailing address, mobile phone number, email address, and date of birth. Mail the entry in a properly stamped envelope to: “Lazy Dog Guest Survey Sweepstakes,” Lazy Dog Restaurants, LLC, 101 Shipyard Way Suite B, Newport Beach, CA 92663. The mail-in entry must be postmarked by September 30, 2026 and received by October 7, 2026. Entries that are illegible, incomplete, inaccurate, postage due, late, or otherwise not compliant with these Official Rules are void. Proof of mailing is not proof of delivery. Entries become Sponsor’s property and will not be returned. Requests for confirmation of receipt will not be acknowledged.

4(c). Email marketing consent and offer. By entering the Sweepstakes, regardless of entry method, entrant expressly opts in and consents to receive promotional and marketing emails from Lazy Dog Restaurants at the email address provided. Consent is not required to purchase anything. Entrants may unsubscribe at any time by using the unsubscribe link in a Lazy Dog email. Survey entrants may also be presented with an offer for a starter on a future visit to Lazy Dog and a welcome email; any such offer is subject to its stated terms and expiration date.

5. Drawing and notification. On or about October 5, 2026, Sponsor will randomly select five (5) potential winners from all eligible entries received during the Promotion Period. Each potential winner will be notified by email using the information provided at entry. Each potential winner must respond within five (5) calendar days and may be required to sign and return a Declaration of Compliance, Liability and Publicity Release. If a potential winner cannot be contacted, fails to respond, is ineligible, declines the prize, or does not timely return requested documentation, the prize may be awarded to an alternate winner selected at random from remaining eligible entries. Up to three (3) alternate drawings may be conducted.

6. Prizes. Five (5) prizes are available. Each prize consists of one (1) $100 Lazy Dog Gift Card to use at any Lazy Dog Restaurants & Bar location. The approximate retail value (“ARV”) of each prize is $100. The total ARV of all prizes is $500. No cash alternative will be provided. Limit one (1) prize per person or household. Odds of winning depend on the number of eligible entries received during the Promotion Period.

7. Prize conditions. Each gift card is subject to the terms, conditions, and restrictions printed on or accompanying it. Winner is solely responsible for all federal, state, and local taxes on the prize and for all expenses not expressly included in the prize. Lost, stolen, or damaged gift cards will not be replaced. Sponsor may substitute a prize of equal or greater value if the advertised prize becomes unavailable, as permitted by law. Prizes are nontransferable except at Sponsor’s discretion.

8. General conditions and release. Participation constitutes entrant’s full and unconditional agreement to these Official Rules and Sponsor’s decisions, which are final and binding. Sponsor may disqualify any person who tampers with the entry process, uses automated or fraudulent means, violates these Official Rules, or acts in a disruptive or abusive manner. Sponsor may modify, suspend, or terminate the Sweepstakes if fraud, technical issues, force majeure, or other circumstances impair its administration. Sponsor is not responsible for incomplete, illegible, late, lost, misdirected, damaged, or technically defective entries or communications. To the fullest extent permitted by law, entrants release Sponsor and its affiliates, officers, directors, employees, and agents from claims arising out of participation in the Sweepstakes or acceptance, possession, or use of a prize.

9. Publicity. Except where prohibited by law, participation grants Sponsor and its licensees the right to use entrant’s name, likeness, hometown, and prize information for Sweepstakes-related advertising and promotional purposes without additional compensation.

10. Governing law and winners list. These Official Rules are governed by the laws of the State of California, without regard to conflict-of-law rules.
